COVID-19 is the illness caused by the SARS-Cov-2 virus, and the test will be investigating possible current infections.
Tests

PCR or Rapid Antigen Lateral Flow Test swab testing
We are using swabs from the throat and, where possible, the back of the nose to detect evidence of the virus, SARS-CoV2. Taking swabs from both locations increases the reliability of the tests. There may be some mild discomfort during the test but this should pass quickly.

Benefits of testing:
Reassurance if negative result. If positive result: minimisation of transmission, contact tracing, early initiation of health promoting interventions. Certificates available for travelling.

Risks of testing:
Nosebleed. Headache. Bruising around nose and eyes. Loss of smell due to swelling. Inconclusive result requiring repeat. 

A previous infection may cause positive results despite not being infectious per se. Risk of false positives and false negatives.

I consent to having the test for COVID-19 by PCR or Rapid Antigen Lateral Flow Test, for which a swab will be taken of the back of the nose and the throat. 

I appreciate the reliability of the swab test is as announced by the manufacturer as advertised on the mpjmedical.co.uk website. This means there is a small chance of ‘false negatives’, whereby a test may be negative despite having the virus. There is also a risk of false positives.

I consent to my personal data, including date of birth, address, email, phone number, booking details and result being held by MPJ Medical Ltd. No other personal data will be held in compliance with GDPR.

I consent for the testing lab to retain my sample(s) and name for a short period to satisfy Public Health England instruction and in line with GDPR requirements. In the case of positive results, I consent to PHE being notified in keeping with national guidance. I will adhere to guidance provided by the NHS:
